We never plan on getting ill, being injured, or needing help. When it happens, Third Coast Health Solutions can be there. With a monthly retainer you have a nurse available whenever you need one.
Monthly memberships provide assurance of care each month.
With the basic membership you’ll receive guaranteed access to 5 hours worth of visits each month. This can include showers, medication set-up, and personal visits along with a range of personalized services.
This monthly membership provides increased access to your nurse. You are guaranteed up to 10 hours per month of visits. It can include weekly shower assistance, medication set-up, along with a range of personalized services.
This monthly package provides you with not only guaranteed hours each month but guaranteed hours each week. Services include showers, medication set-up, along with a range of personalized services.
Membership Package Information
Possible services available with a monthly membership include: weekly medication set-up, routine wellness check-ins, vital sign and blood glucose monitoring, weight monitoring for heart conditions, phone calls to providers, phone calls from you to me with questions, appointment preparation, appointment attendance, education sessions, care coordination, chronic illness management assistance, medication pick-up service, medication education, after-visit summary reviews, assist with transition post- hospital or rehab stay, nutrition information, communicating updates with family and caregiver, health document organization, community resource assistance, and more.
Services Provided
While a monthly membership does provide priority status is does not provide emergency status.
Situations such as falls, injury, shortness of breath, chest pain, low blood sugar, etc require 911 or an urgent phone call to your doctor, not your nurse concierge.
This does NOT replace emergency care
Scheduling
Services are preferred to be scheduled a few days ahead of time but having a nurse concierge means some flexibility is afforded.
Any same-day visit requires at least a 2 hour notice. Some services can be set up after business hours, but it must be discussed with the nurse ahead of time.
This is a membership package, therefore it is a recurring monthly charge.
If at any time you choose to cancel your membership, provide a 4 week written notice. No hassle. No guilt.
